Reablement Social Work Practice Lead

What you’ll be doing

As Reablement Social Work Practice Lead, you’ll play a key role in supporting adults and carers to achieve positive outcomes, maximise independence and access the right support at the right time. Working within a multi-disciplinary Reablement and Independence Service, you’ll provide professional leadership, guidance and supervision to Reablement Social Workers, while also holding a small caseload of more complex work.

You’ll undertake strengths-based assessments, care and support planning, reviews, risk assessments, Mental Capacity and Best Interests assessments, ensuring practice is person-centred, legally compliant and focused on promoting independence. You’ll work closely with therapy colleagues, locality teams, hospital social work teams and external partners to support joined-up working and smooth transitions where longer-term care needs are identified.

A key part of the role will be improving and developing social work practice within the service. You’ll support evidence-based approaches, contribute to audits, provide advice on complex cases, support safeguarding activity where required, and ensure accurate records are maintained so residents and carers receive high-quality, timely and effective support.

Annual increments, where applicable, are usually payable on 1 April, subject to six months’ service and satisfactory performance, until the maximum spinal column point is reached.
