Care Home Liaison Nurse - Older Persons Mental Health Team

The Role
An exciting opportunity has arisen to join the Older Persons Mental Health Liaison Team within Inverclyde Mental Health and Wellbeing Services. The team provides specialist mental health support to care home residents, acute hospital patients, people aged over 65 experiencing mental health difficulties, and individuals of any age living with dementia. Working alongside nursing, medical and psychology colleagues, you will deliver high quality, person centred assessments and interventions, while supporting service users, families and carers. The post also offers opportunities for professional development, particularly in Psychological Interventions in Dementia (Stress & Distress). As this is a secondment opportunity, applicants must have approval from their line manager prior to applying.

Fixed Term Role - Maternity Cover (9 Months - May 2027)
Argyll Unit/Larkfield Unit (Greenock)
18 Hours Per Week (9am-4.45pm Mon-Fri 9pro rata/days to be negotiate)
Key Responsibilities
The postholder will work autonomously as part of the multidisciplinary Older Persons Mental Health Liaison Team, managing a clinical caseload of patients with complex mental health needs. Responsibilities include assessing, planning, delivering and evaluating care; undertaking mental health and risk assessments; providing short-term interventions; supporting care planning and service transitions; and facilitating early discharge and admission avoidance where appropriate. The role involves working closely with service users, carers, care home staff, hospital teams, GPs, consultants and partner agencies to ensure coordinated, person centred care. The successful candidate will provide specialist mental health advice, education and support to healthcare professionals, care home staff and students, contribute to multidisciplinary meetings and case conferences, maintain high quality clinical records, promote evidence based practice, support service development, and play a key role in safeguarding, risk management and continuous improvement across older adult mental health services.
Knowledge, training, qualifications and/or experience required to do the job:
- First level Registered Mental Health Nurse (RMN)
- Current NMC registration (PIN required)
- Relevant post-registration qualification (e.g. BA, BSc or Diploma)
- Robust clinical experience at Band 5 or above
- Experience of working with older people experiencing mental health problems and/or dementia
- Preferably experience of autonomous/lone working
- Evidence of continuing professional development
- Excellent communication and organisational skills
- Ability to undertake comprehensive mental health and risk assessments
- Knowledge of relevant mental health legislation, policies and procedures
- Ability to manage a caseload independently and work effectively within a multidisciplinary team
- Full UK driving licence (Essential)
